I think the confusion comes from expecting the medication to produce an obvious sensation. But sildenafil tablets aren't an aphrodisiac. You don't necessarily feel a sudden “switch” turning on. Sexual stimulation is still necessary, and the noticeable difference may simply be that an erection becomes easier to achieve or maintain.
That means someone could take it, feel completely normal, and still find that it worked when intimacy actually begins.
There are other things that can affect the experience too—anxiety, expectations, timing, food, alcohol, and individual response.
And if the first attempt isn't successful, that doesn't automatically mean the medication is ineffective for that person.
